B Treatment Benefit (TB)
   
  Treatment Benefit is to treat the same Crisis Event within 2 years from the date of the first confirmed
diagnosis of the covered Crisis Event.  It is on reimbursement basis.  Treatment Benefits include in-hospital surgical procedures, blood tests/products, in-hospital physicians' visits, in-hospital drugs, out-patient medical consultations, X-rays, chemotherapy, radiation therapy, haemodialysis and any other related treatment by a legally qualified physician as defined in the Policy Contract.
   
C Daily Hospital Benefit (HB)
   
  Hospital Benefit provides daily hospitalisation benefit for each day the Assured is hospitalised for a minimum of 6 hours due to covered Crisis Event up to a maximum of 180 days within two(2) year from the date of the first confirmed diagnosis of such Crisis Event.
   
D Emergency Medical Evacuation Benefit (EMEB)
   
  When the Assured has any serious illness or injury while he/she is travelling outside Malaysia, American International Assistance Service (AIAS) will provide en-route medical care and transportation as it deems necessary to move him/her to another location or back to Malaysia for medical treatment.  The Company shall pay directly to AIAS the covered expenses for such evacuation up to the maximum of EMEB's sum assured applied for.
   
E Free American International Assistance Service (AIAS) membership
   
F Premiums are payable continuously up to age 54 years of the Assured.
  Coverage shall continue until the Assured reaches age 70 years at the same benefits & sum assured less any amount paid for heart treatment (i.e. angioplasty or coronary laser/atherectomy).

3 Underwriting guide:
   
 
3.1 Issue age limit:  6 months - 54 years (coverage continues to age 70 years)
   
3.2 Level premium at entry age: Strictly no backdating of policy effective date.
   
3.3 Renewal Bonus of 5% for the 1st 5 renewal years on the sum assured for crisis event subject to a maximum of RM300,000 per life including renewal bonuses for all Healthguard policies with the Company.
   
3.4 Elimination period for coverage on Crisis Events:
   
(a) 90 days from Policy Effective Date or Date of Last Reinstatement, whichever is later.
(b) For Female Cancers which means Female Beast Cancer and Female Genital Cancer, the Waiting Period is 180 days from Policy Effective Date or Date of Last Reinstatement, whichever is later.
   
3.5 Premium mode:  Annual, Semi-Annual (x 0.51) and Quarterly (x 0.27) only.
Minimum modal premium is RM50.
   
3.6 Eligibility:  Malaysians (occupation class:  1,2,3 & 4) and foreigners (occupation class:  1 & 2 only).
